How do I fill out DA 67-9-1 form?
There are several fields for filling out: 1) rated officer identification; 2) authentication of the raters; 3) verification of face-to-face discussion; 4) description of a duties and responsibilities of a rated officer; 5) indication of major performance objectives; 6) list of contributions, skills and future assignments for which the soldier is best suited. The entire document is filled from the words of the officer by the members of the evaluation commission.
