Who needs a RMV-1 form?
This application form is used by the owners of the motor vehicles in Massachusetts for various purposes. The owners may need it, for example, when they sell the motor vehicle and the registration is transferred to another owner. In general, this form is required when any changes are made to the registration, insurance or plates of the motor vehicle.
What is the RMV-1 form for?
This application form is used for several purposes: to apply for salvage of the motor vehicle title; to apply for a title only; to apply for a registration only; to apply for a registration and a title; to apply for a summer/winter swap; to apply for a registration transfer; to apply for a title add registration; to apply for a registration transfer by a surviving spouse. The information in the form is used to register or re-register a motor vehicle and to provide information about the vehicle’s owner.
What documents must accompany the RMV-1 form?
Depending on the application purpose, the car owner may be asked to provide additional documents together with this form.
How long does it take to fill the form out?
The estimated time of completing the form is 15 minutes.
What information should be provided in the form RMV-1?
The form has two copies: original one and the copy for the registrant.
The applicant has to provide the following information:
- Purpose of application
- Information about the motor vehicle (plate type, registration number, identification number, year, make, model name, body style, color(s), etc.)
- Information about the owner (name, address, SSN, date of birth, sex)
- Title’s information
- Information about the lien holder
- Insurance certification
- Sales or Use tax schedule
The owner, the seller and the dealer have to sign and date the form and type all the required information.
What do I do with the form after its completion?
The completed form is filed with the Massachusetts Department of Transportation.
