I agree to the terms and conditions of employment. In most of the above examples, each NURSING/DESIGNEE signature is signed and recorded in the hospital's official personnel record system. However, because this information may not be available to the staff and family members, the nursing staff signatory may have a second personal signature recorded, as indicated in the example of a nursing staff member. If the individual signs their name before completing “SHIFT #,” then a shift change-up process must be followed. The shift-up process is a mandatory event for RN's. The nursing staff must schedule changes to their assigned shifts and the shift-up process must be completed in order to keep their position. The shift change-up process is part of the annual personnel review. Scheduling shift-ups for all staff and family members must be done through the staffing department. These processes are part of the monthly staffing review and a team of nurses must plan the next shift-ups during the next regular staff meeting. After the next monthly meeting the department must hold an emergency meeting in order to complete and reschedule additional shifts. Example: SHIFT 1 Hospital 1 is required to have a nurse-designee and a nurse-staff member assigned to each shift, but only one nurse-designee is certified on a given shift. During each shift, the nurse-staff member will have to complete a shift-up for each shift before leaving for the next shift. The nurse-designee is assigned a shift. When required, each staff member must complete the shift-up assigned to him at the end of the previous shift. The nurse-staff member is scheduled to work 1/1 1/1 1/1 until 1/15 1430. The first time this nurse-designee leaves the hospital, and no longer meets the qualifications of a nurse-designee, another staff member can be scheduled as a nurse-designee for that shift. SHIFT EACH DAY The next daily shift will have an overnight shift (1:30-4:00 AM), which should be scheduled during an earlier nurse and family member meeting. The nurse and family members are expected to attend the overnight shift meeting. The evening shift starts at 6:00 PM and ends at 6:00 AM the next day. Once a nurse or family members leaves the hospital, the nurse-designee must return to the morning shift for the night shift.
