A new contract pharmacy may register and add these items on the Online Registration Form (found at the end of this Guide). When a contract pharmacy registers, it may use one of three methods to register new medicines: 1) Select a pharmacy from the Registered Pharmacists, Registered Patient Nurses, and Registered Health Clubs list in the Schedule of approved Pharmacies in Part 2 (available from the New Pharmacy Forms section of this Guide). 2) Use a form provided by the Office of Patient Records on the form and return it with the original of every prescription from every registered doctor in Canada to the Ontario Prescription Monitoring Program Office at the Ontario Ministry of Health and Long-Term Care. 3) Submit online the contract registration form using a computer at the site of the provincial pharmacy board. If both of these methods are chosen, the contract physician will sign the contract physician's contract with the contract pharmacy and the contract pharmacy is entered into the contract pharmacy database. When a contract pharmacy registers a supply list, a list of all the supplies required to fill the contract pharmacy's prescription is required. Before a contract pharmacy registers a supply list, a contract physician will provide all the following: 1) A list of the supplier's names, locations, and telephone numbers; 2) The names of all the contracted pharmacists who will be working for the contract pharmacy, listing the contract physician's name and telephone number; and 3) Names and telephone numbers of the contract pharmacists working on the supply list, listing the contract physician's name and telephone number and the contract physician's signature for the contract.
