(Note: This information only needs to be included if you are not receiving a phone number on the source protection areas. If for some reason you are not, you can remove this information.) 8) If a business is located in a source protection area, a copy of the Business License or other required business information. (Optional) 9) Whether the recipient is subject to and required to be licensed in the county where the recipient is located. You have 24 hours from the date of mailing to correct this information in the same manner listed in section 2 above. Your request and all information should be sent by certified mail, return receipt requested. The return receipt must show the original signature of the person from whom the mail was sent. Any attempt by a water district or other source protection agency to modify, amend, or change this information after the mail has been mailed is grounds for legal action in municipal court. (Please allow seven or nine days for an item to be received.) The fee for the source protection area is 50.
