For months, they tried an unproven new chemotherapy drug that made the body's immune system attack only cancer cells. Some patients fared better, but some fell into severe seizures, heart attack-like angina, kidney damage, and infections. One of Brett's doctors was surprised to learn he had been able to enroll only five patients at the hospital so far. The treatment cost 2,000 a month and had an extremely poor safety profile. “I am astonished how difficult it's been to get many patients at one site to get on one chemo, even after they've already received a great deal of education,” says Dr. Steven L. Goldstein, who was one of the lead researchers for the drug's approval in the fall of 2002. “But there was the realization that, even at the lowest doses,” the new drug offered “an enormous improvement over standard therapies.” Since then, more than 50 tumors tested by the US Food and Drug Administration—including those of Brett—have shown dramatic benefits from the new drug. “What we don't want to do is say that there will be a few that don't respond,” says Goldstein. In June 2007, a year into the trial, the FDA announced that it had approved the use of the drug for most people with the disease. More patients can now get new drugs approved for a wide variety of cancers. In a series of letters that Goldstein wrote to the agency as a special consultant, he argued in favor of a “much broader application” of this approach. As Goldstein saw them, patients are often given drugs that treat symptoms—such as weight gain or fatigue—instead of the disease itself. But the new approach, combined with a greater understanding of the fundamental biology of cancer, led the FDA to approve the drug for nearly all patients with advanced tumors, he notes. “We are talking about millions and millions of patients,” says Goldstein. Now the FDA is studying whether the same approach might help people with other types of cancer, including melanoma and breast cancer. Goldstein says his group has no firm conclusions about when that will happen. But researchers at the Dana-Farber Cancer Institute in Boston are testing more than 250 patients with liver cancer. “That is what the real testing for [this] will be,” says Dr. Mark D'Alisa, director of the laboratory that is working on it. So far, he says the response in these patients and the response in patients with other types of cancer has been dramatic.
