You will be required to be examined by the Board of Cosmetology to determine whether you are a candidate for licensure. This license will not be valid or enforceable beyond January 1st, 2012 unless the Board is convinced that you are not a candidate for licensure within two (2) years of the date of receipt of the application. As of October 19, 2010, the Board is not satisfied that you meet the criteria for licensure as a temporary tattoo Artist and has declined to approve your application to practice. The license will be automatically suspended upon receipt of notification of refusal. As you are currently under suspension, this license will be automatically withdrawn without further notice to you. It is recommended that you attend the next board meeting or call the Board's main number for further instructions on procedures which you must follow in order to avoid suspension of your license. Please let me know if you have any questions regarding the license, including your personal circumstances that are relevant to your application. Please allow eight (8) weeks for processing of your license application. If you are unable to attend a board meeting, please contact your state licensing authority directly to schedule an appointment to have an examiner examine you. Your license will be mailed to you.
