When you are ready to renew, you will be required to fill out the renewal form. If your Polio Regina status is “Active”, you may renew automatically at least one month prior to the renewal. If your status is “New”, you may renew at any time prior to the renewal. If there are any issues (such as you have lost your photo ID) or the information on your renewal form is inaccurate, then you must renew. All information must be correct when you submit a membership application. If the information you provide on your application is incorrect, you may re-fill the form in its entirety before sending it to the office. If you have any other problems, then you must call the office, or email the office. If you have not made all the required payment(s) toward your membership, you may call the membership office. The payment must be sent by regular mail, not by check or direct debit. To receive a refund of fees and/or interest, you must mail or email to the office your completed application form and return postage prepaid to the address below. The office receives applications at least once per month. Please be sure to include the name and membership number of the person you are renewing. Renewal forms MUST be received no later than the last day of the month prior to the month in which the renewal will take place. You will be required to pay all the fees that you owe prior to renewal. We do not require any previous payment on your application, and you do not need to send money by check or direct debit. When we receive your completed application, we will ask you to send your name, age, and mailing address to the address below. (If a member who has recently been diagnosed with Polio, please specify, so we can correct any errors before the renewal begins.) Polio Regina Incorporated. Box 988, Regina, SK S7K 3B6 You do not need to renew your membership if the registration fee (for current members) or the renewal fee (for those needing it because of recent poliovirus symptoms) have already been paid. This fee may be paid either in full by cash, debit, credit card (up to 40, please), or bank draft.
