Any material information about the applicant regarding their business transactions, including the nature of any real estate and any sources of income. E. For units with water heater, the name, address, and telephone number of each water heater in the building. A. The applicant's name and address must be the exact name, address, and telephone number on the permit. If the applicant has any other business address other than the City of Grantee, the applicant may provide a copy of the letter and letterhead of any public or private corporation, limited liability company, partnership, limited liability partnership, or other legal entity that the applicant owns, including its local office, and that does business in Grantee. B. If the applicant has any other business address other than the City of Grantee, but the applicant has furnished a license number to the City of Grantee, the applicant shall include that number in the applicant's name in the application. C. The applicant shall certify in writing that he or she has complied with all requirements and restrictions imposed under the Federal Flood Insurance Act and the Federal Nuclear Energy Act, both under the Public Utility Registration Program, and the Federal Water Pollution Control Act, and that the applicant has a valid license issued by the Federal Government authorizing the issuance of the permit. D. The applicant shall attest that all information is accurate, whether the applicant is required to give a receipt for the application. Applicant shall certify that the applicant has been inspected by one (1) registered design professional, certified under rules adopted by the Town, in making its design judgment request. Applicant shall provide evidence of such inspection in a form satisfactory to the Chief of Engineering and the Chief of Buildings.
