The employee or N/A employee may also request the following information as evidence of their employment status: (A) Social Security Number of individual (2) Date of Birth/Birth Certificate or (3) Taxpayer Identification Number of individual who provided social security number. Social Security Number may be provided as a paper copy of an SSN. If the information in this report has not already been on file with CDS prior to August 1, 2004, information relating to the individual's prior employment status or prior wages will be included on a “Notice to Employee” that the individual must review and sign before completing the CD. The Notice to Employee is required for those individuals who are not currently employed and who were employed in a non-salaried position prior to the month they received a notice to start a wage payment or before the month the wage payment begins.
