Although the rates in youth are declining, only 2.5% of Malaysians of the lowest socioeconomic status (SES) and 1.4% of adults smoke daily.[1a] The socioeconomic development level of a province or an urban neighborhood has a substantial effect on the proportion of adolescents smoking, particularly in rural environments.[1b] The socio-economical environment of a locality is considered to contribute to a community's level of tolerance and tolerance for smoking and a consequent positive relationship with the government. Rural socioeconomic status is defined by the proportion of the population that is under the national poverty line (PNL) as defined by Statistics Malaysia with a household income of less than RM250,000 per annum, and the age distribution of the population. Furthermore, socioeconomic status plays a key role in smoking initiation among youth in Malaysian Malay community.[2a] However, the prevalence among Malaysian youth is higher among males than females.[2b] Reasons for the high smoking rate among males have been attributed to factors such as social norms and the use of nicotine patches and other substances to self-medicate tobacco withdrawal.[1c] Tobacco is a major global cause of non-communicable diseases, with 1 in 4 (40%) people worldwide living with diabetes, 1 in 3 (32%) people developing heart disease and stroke, and 1 in 2 (25%) women dying prematurely from smoking-related causes.[1d] In the United States, a recent study estimates that over 80% of smokers are men, 80–90% have at least some college education, and the average smoker consumes 7.3 cigarettes per day.[3] Youth smoking is a major problem among Malaysia due to the high proportion of males who consume the most tobacco.[1e] The tobacco control program focuses on the promotion of safe tobacco access but cannot prevent an adolescent smoker from using tobacco in the absence of health concerns, which are associated with socioeconomic status, gender, and age.[4] This is in contrast to the case in the US, where high youth smoking has been linked to the relative success of the campaign. In Malaysia, there has been an increase in the smoking prevalence of youth in recent years, especially in rural areas.
