For the first three months of your membership it is your responsibility to provide CVS/pharmacy with the above forms. To get your medical records processed within five business days, we suggest you send them directly to: Benefits and Services CVS/pharmacy, 4015 W. Main St., #202, Miami, FL 33155. CVS/pharmacy does not charge for this service. The cost is 15 per page. For medical service fees, we would suggest you contact CVS/pharmacy's Customer Services Department directly at. This is an important fact to note that may be overlooked when searching for a CVS/pharmacy member. The cost of the benefit is based on a monthly membership in the benefit plan. There are limited exceptions, most usually for a CVS/pharmacy member with family members enrolled in other benefit plans.
