In the event of loss/damage from fire / accidents / disease / injury caused by any of the above nominees within the period mentioned above, the remaining amount will be paid to the person/s mentioned in item 7 of this form and a full statement of this case will be presented to the Election Commission if the candidate is to be elected. In case you have a valid residence certificate issued by the District / Provincial Government (except for Lahore / Islamabad) from the Department of Posts of Pakistan which clearly states that the applicant is residing and is entitled to vote in this election, you can nominate the following persons for office of President, Vice President and/or District Magistrates (if any) (in the office) or any of the above candidates by forwarding a photocopy of his/her valid residence certificate along with this application form to the office of the District/ Provincial Government immediately after your submission of this application form. The application form must be accompanied by a photocopy or a scan of the voter's identity card issued by the District Election Commission in the year to which these candidates relate. When applying for the nomination of a person with disabilities from the Department of Posts or the Department of Social Welfare and Development, you have to provide a photocopy of his/her passport (or an extract of his passport if a medical certificate has been issued) issued in the country(s)/regions or State(s) to which the person is a citizen.
