The Awards Committee reserves the right to change, rescind or extend dates if the application is incomplete or if the nominee is ineligible. 4. To make an award, an award document should be sent to the email specified on the nomination form and should include the following items: (a) The nominee's name, title, department, campus address, e-mail address, telephone number and a brief description of the individual's contribution to the university community; and (b) the nominee's name, title, department, campus address, e-mail address, telephone number and an explanation of why the person is worthy of an award. It is the responsibility of the nominee to submit all of this information in their application. (Please see guidelines for the nomination form.) 5. All nominations will be reviewed by the Awards Committee. All recommendations for awards that are approved by the committee will be honored if the nomination is completed and signed by at least three nominees. For all other awards, the entire process will depend upon the recommendation and judgment of the Awards Committee members. 6. A nomination will be deemed accepted if the nomination is received in its entirety and any part of it is signed by at least three nominees.
