Each risk area is separated by a line; you enter the hazards on the right-hand side. On the left, you enter the risks that are currently a concern in the risk area to which you want to address. You can adjust the values for the hazards on the right, and in the left-hand column, set up some basic questions to answer your questionnaires. For example, you might want to know if there is a hazard that concerns you but is not yet a concern in any particular area. The questionnaires on the other hand might be more concerned with areas that are under your control that actually are hazards; you can then respond to these questions before deciding what actions you should take in that particular area that may involve risk mitigation. The risks are also indicated on the form for which you are asking to have these or other hazard mitigations (to do something about the risk). For example, the hazards that you are trying to reduce or eliminate are not listed on the form.
