An applicant seeking to initiate a voluntary separation from employment shall provide the following information when requesting a voluntary separation: (1) Employee's complete name, as it appears on the employee's most recent state driver license or passport; and (2) Date of birth; and the employee's marital status shall include the following: (a) Is married; or (b) Has been married; but has separated; or (c) Now is separated; and (d) The date of separation. (a) Except as otherwise provided in this section, the notice of intent to resign required under paragraph (g)(2) of this section must be provided no later than the first day of the first pay period following the date of voluntary absence. (b) The notice shall state that the employee is entitled to a period of paid vacation, for the purpose of seeking work.
