Comments and Help with printable esa letter

As such, please provide a brief description of what benefits the ESA would provide, what limitations the ESA would not provide, and the anticipated timeline to implement the ESA in the residence hall, including how long the person will be able to use the ESA before his/her accommodation or change in behavior becomes required. All materials regarding the proposed ESA must be submitted within the stated time frame. Failure to provide all materials requested will result in the student(s) being required to submit additional documentation via the online waiver form. Students are required to complete the online waiver on a single screen, and are not required to complete the form all at once. The online waiver process allows students to upload and complete the online form of the request on their own time. The submission deadline is Thursday, December 31, 2018. All documentation must be submitted to: Emotional Support Animal Request for Documentation, Trinity College 300 Summit Street. Please include a copy of the student's health care information, as well as any documentation provided by the Student to confirm that you are authorized to request the requested documentation (i.e., a doctor's note or letter in support that the student will need to access health care services.) All requested documentation must be received by Trinity College for consideration by the Student's Faculty Advisor by Friday, March 31, 2019, in order for a waiver to be granted. A student should check with his/her faculty advisors to determine if any additional evidence is required. If a medical emergency arises that would require the accommodation of an emotional support animal for a faculty member, such as illness or trauma (e.g., a student is fighting for their life) or the imminent threat of suicide, an instructor may request that the student provide a medical emergency waiver on an emergency basis. Upon obtaining this waiver, the student will be issued a valid ID for the purpose of entry into the residence hall. No waiver will be granted for personal or emotional reasons, or without an additional physician's note. All requests for the emotional support animal waiver must reference the need for an emotional support animal in the specific student's case. No other conditions may be used to request an emotional support animal waiver — for example, “I need an emotional support animal for my husband to get to work and to give me peace of mind.” If the student has any questions about the emotional support animal provision of the accommodation, they should contact their faculty advisor.
