How do I fill out this form?
If you are filing a request on behalf of a bank, you need to fill out points 1 through 8 and have the applicant for the loan (the tenant) sign Item 9. In this part you need to write the name and address of your institution and the name and address of the landlord who leases the apartment to the tenant. Give the address of the property that your applicant is interested in. After that write the name and address of the applicant next to his signature.
If you are the landlord who received this request, your job is to fill out Part II. You have to specify the dates of the verification period, the amount of rent per month and how many payments you received. Separately write the amount of late payments. In conclusion, inform the bank representative if you are satisfied with the tenant.
