An individual requesting to immigrate to the United States must possess one of the following: an original birth certificate, or an attestation on the appropriate document from the Haitian consular section; a document with the following characteristics indicating that the person's parents were born in Haiti: a) a birth certificate issued by a Haitian authority; b) a document signed by the parents stating that their parents were born in Haiti; c) a document evidencing two or more generations of parents who were born in Haiti; or d) a document validating an address in Wait a. The document must include a number that uniquely identifies the individual and their birthplace; or, if the individual is not in Haiti, it must have been generated by the Consulate in that country.
