How do I fill out Form SF 86?
All information must be typed or printed in ink. When filling out the form refer to the previous Form SF 86 to compare if anything has changed. Provide only new information. For additional information a filer must use separate sheet of paper where they need to enter their SSN on top of the page. Form 86 is two-page document divided into four blocks:
- Block 1 stands for identification information including individual’s name, date of birth, SSN, and address
- Block 2 accounts for questionnaire about individual’s professional experience, education and private life. All YES answers must be explained in Block 3
- Block 3 is for explanations that an individual must give to all YES answers in the previous block
- Block 4 contains certification statement, signature and date field.
