Who needs the Schedule O (Form 990 or 990-EZ)?

Schedule O (Form 990 or 990-EZ) is an addendum officially called the Supplemental Information to Form 990 or 990-EZ. It should be completed by tax-exempt nonprofit organizations that file their tax returns using the IRS Forms 990 or 990-EZ.                 

What is the purpose of the Schedule O form?

A nonprofit organization is supposed to be using this IRS Supplemental Information Form, rather than separate attachments, to provide the IRS with sufficient information for responses to specific questions on Form 990 or 990-EZ, and explanation of the organization’s operations or responses to various questions.

In this way, all the information that refers to the tax return of the tax-exempt organization, but cannot be revealed on their IRS forms 990 or 990-EZ, is covered on their Schedule O.

Which documents does Schedule O support?

As has been stated above, Schedule O (Supplemental Information to Form 990 or 990-EZ) form serves as an accompaniment to the tax return of a non-profit businesses using IRS Forms 990 or 990-EZ.

When is Schedule O Form due?

The US Internal Revenue Service expects both Forms 990 or 990-EZ to be filed together with their comprehensive explanation, which is Schedule O. The due date for submission is always the standard 15th day of the fifth month of the fiscal year. Typically, it falls on May 15, if it is not a federal holiday or a weekend.

What information should be provided on the Schedule O Form?

The very first items that should be indicated legibly are the name of the tax-exempt organization and its employer identification number (EIN).

The rest of the form is actually a blank form that should be filled out as the filer’s situation requires. However, it is of vital importance to indicate clearly the number of the item from 990 or 990-EZ form being answered.

What do I do with the Schedule after its completion?

The completed form is attached to the 990 or 990EZ and filed with the IRS.

Welcome to my series of video tutorials on how to prepare form 990-ez information return for small tax-exempt organizations I'm David McCree I'm a CPA and you can find me on the web at my form 990-ez comm part 3 on page 2 is required to be completed by all 501c3 and all 501 C 4 organizations it's optional for other types of organizations like a 501 C 7 social club for instance or a 501 C 6 business League and others part three is where the organization shows how much of its money was spent on its mission most organizations don't put nearly enough effort into this part of the form when in fact it might just be the most important part of the form 990-ez only money spent in the direct conduct of the organization's mission is shown in part three lines 28 through 32 management and general expenses and fundraising expenses are not shown here at all so unless you did not have any management in general or fundraising expenses line 32 will not be equal to line 17 which is total expenses on the page 1 of the 990 easy there's not much room provided in this section that's part 3 to accommodate the detailed descriptions of your programs in most cases it's desirable to simply enter C schedule o like I've done here and enter the descriptions on schedule o if schedule o is used be sure to put a mark in the box provided at the beginning of part 3 to indicate that the organization has used schedule of to provide answers to this part so people will know where to go looking for the explanation the first question in part 3 asks what is the organization's primary exempt purposes given for an answer so you'll almost certainly need to enter C schedule o and provide an answer there on schedule o you may be able to fit the answer on the line provided if you have a very simple organization but even then it's not really recommended to do it that way for instance to help homeless persons might be acceptable but to provide hot meals medical and dental care and addiction treatment and counseling to homeless persons in the downtown st. Louis Missouri area that would be a much more useful description to somebody who might be considering making a donation to your organization lines 28 29 and 30 provides space to list and describe your three largest programs activities most small organizations will lump all their activities together on line 28 since they're usually fairly tightly focused on one main activity but if your organization has several different activities and you track the income and expenses separately for each activity you should probably use a separate line for each program for instance if you're a local wildlife conservation organization and you provide education to schools and other groups and you also have a couple of events each year to increase awareness of your organization and distribute educational materials you could probably just group all the activities together under one description on line 28 however if your organization helps homeless people by...
