Who needs a form FW 001?
Form FW 001 is a request to waive court fees. If you have low income and need money to pay for your household, the court may waive some court fees after processing this application.
What is form FW 001 for?
In order to apply for a reduced court fee, you need to file this form to prove your eligibility to the court. However, if your civil case is settled for $10,000 or more, you still have to pay.
Is form FW 001 accompanied by other forms?
This form is an application that illustrates your financial situation to the court. However, you have to provide other documents that prove your statement: for example, your bank account details or your tax declaration.
When is form FW 001 due?
Usually the court clerk gives a fee waiver immediately after receiving the application. But there are cases when court has to analyze your application.
How do I fill form FW 001 out?
Insert general information about yourself and your job. Write about your lawyer and check in the box if he agreed to advance all or a portion of your fees or costs. Choose what fees you ask the court to waive and why. There are three reasons in the application. Check in the box if you receive public benefits, like Medical, Food Stamps, SSI; or if your gross monthly income is less than given in the table of possible variants; or check if you don’t have enough money to pay for your basic household needs (if so, you have to fill out page 2). On page two you have to specify your monthly income, household income, what money and property you possess and your monthly expenses.
Where do I send form FW 001?
After completing this form, mail it or deliver it in person to the court clerk at the court where your case was heard.
