All renewals should include the date of renewal, licensee name, physician's last name and address. No new renewals will be issued by May 14, 2004. Failure to submit an application will result in a license renewal being denied. Failure to retain the original license renewal application within one year of its issuance will result in a license transfer being withheld for all subsequent violations. The Board staff will inspect the applications on June 17, 2004. All renewals to medical doctors requiring renewal are available at the office of the medical staff. Medical doctors requesting application information must have a current license. Applications for renewal must be signed in the presence of at least one Board person; any person not designated by the licensee to sign the renewal application may sign only affidavits. No license shall be transferred without the written consent of the physician to whom it was originally issued. The license may be issued to another physician and the application fee may be refunded at any time by the licensee. If no person can be found to sign a renewal application, the Medical Council will issue a letter to the licensee that includes all the necessary information in order to obtain a license. Physicians whose names are not listed above will have their applications mailed to them. No action is necessary by the physician or the physician's attorney to verify the name or address listed on the application. There is no fee for renewal. Licensees who fail to renew their license within thirty days of the license expiration date will be considered in good standing for the subsequent ninety days. However, they will not be issued a new license. An application fee of 1,250.75 will be refunded to applicants who request a new license. If an unsuccessful renewal is requested, the Medical Council will mail a letter to the licensee which informs the physician that an application has been mailed for license renewal. The license will not be renewed by a physician who has had six unsuccessful renewals. A letter will be sent to all physicians that the Medical Council has determined may not renew their licenses through fraud or misrepresentation. A person whose license has been permanently revoked by the Medical Council shall be removed from the list in a timely manner and the Medical Council will be given the right to review his or her application before a license may be renewed. No new license shall be issued unless the former or existing physician has been notified, within 30 days of the date of the revocation, that the physician's license has been permanently revoked by the Medical Council.
