Application forms can be obtained from: Medical Professional Practice Act-Licensing Board-MSB Online Application; by visiting the EMS-500 website and completing the online application; or by visiting any of the following: EMS-500, EMS-600, EMS-601, EMS-602 (for an examination), EMS-603 (for an examination and endorsement), EMS-604, EMS-605 EMS-601-602 (for an examination and endorsement) or EMS-603-304 (for an endorsement). All medical residents are required to pass an examination within 8 years prior to obtaining licensure. All examinations are conducted at the request of the license examiner. Examination results may not be used for licensure. An applicant holding a license to practice medicine in Michigan that is less than 2 years old may file an application to practice medicine in Michigan with the American Board of Medical Specialties, the Missouri Medical Board and the Federal Emergency Medical Board. Applications and accompanying documentation must be submitted to the licensing officer prior to the proposed start date of the license application. The fee for this application is 60. This application is a non-refundable application and an applicant will be charged a 60 non-refundable fee. The application deadline is July 9, 2017.
