(a) (1) Any person liable to PAY or to FICA taxes (a) (A) for whom an assessment has been levied pursuant to this section may file in the office of the Commissioner of Revenue an application for a credit against those taxes in the amount of the credit allowed, pursuant to the provisions of sections 23, 30C, and 30D of chapter 2. The assessment must be based upon a reasonable estimate of the amount payable in relation to the business conducted by the entity. The assessment must be in a form prescribed by the Commissioner of Revenue. (See instructions for Form 14B.) The application shall set forth the name and address of the entity, its business name or trade name, and the addresses of its principal place of business, the names and addresses of each owner of 5% or more of the voting stock and the amount of each exemption claimed or allowed. The application shall also set forth the number of the assessment, the date and manner in which it was made, the name and address of the person filing the application, the names and addresses of both the entity and the owner(s), and the total amount in respect of which assessment has been made.
