When is Form GC 251 Due?
Being the guardian, the court requires you to fill out, sign and submit this form no later than thirty days after the anniversary of the day when you have been appointed as the guardian. The status report must be authorized by the Probate Code section 1513.2.

What Information do I Include in Form GC 251?
First you must provide the details about the court: its street and mailing address, city and zip code and the branch name. After that indicate the information about the guardian: name, present address, telephone number, health condition. Then provide the information about the child under the guardianship: name, place and date of birth, education, grade, physical and emotional health, all persons in the child’s home, child’s natural parents, need for guardianship and the conditions of guardianship. The form must be signed by all parties.
