Great Product! I love this software; it has been invaluable. I have used it to fill out job applications, insurance, medical & school documents. I'll continue to use it. It is very easy to upload documents to this software. Editing is user-friendly even for those not technically savvy. I like that I have many options in saving or exporting my finished document. Also, I like that my previous documents are saved because there has been more than once that a document has been lost or needed again. I have been saved by the fact that PDF Filler has the previously used document saved. I have not found anything I really dislike about this software. I'll be honest in that I'm not crazy about the price but I have found this software so helpful I have been using it for over two years now. I've recommended it to several people. I did not rate the customer service because I have never used it. I guess the fact that I've been using this for over two years & never had a customer service issue speaks for itself.

GREAT product!
GREAT product! Nothing negative! I draw up a lot of contracts for my job and this website makes it much easier. This is very easy to use, the interface is very clean. I wish there was an app to support this product. A lot of times I am on the go and I need to fill out PDFs. It would be nice to have an app.

The only acceptable proof of profile for a Soldier to produce is a Department of the Army Form 3349, Physical Profile, not a sick call slip. The profile is honored only if the individual has the document on hand and can present it when asked.
A DA Form 3349 will be used by the United States Department of the Army. It is known as a Physical Profile form and it will be used as a way to keep a record of a soldier's physical profile. This includes their physical fitness level and their physical capabilities.
The physical profile serial system is created around the various military duties and provides a numerical standard that a medical doctor recommends during the routine recruit physical. These physicals are based primarily upon all of the body system functions.
Visit http://www.uscg.mil/forms/ To access their Forms. Peruse a database of available OPNAV forms for Navy and Marine Corps users. Some forms can be filled out prior to download or printing. Lists all forms for the National Guard Bureau (NGB), the Air National Guard (ANG) and Army National Guard (ARNG).
If the temporary profile exceeds 6 months, the Soldier will be directed to a specialist who can extend the profile to 12 months, change the temporary profile to a permanent profile, or refer the Soldier to an MEB if they determine they do not meet medical retention standards. Temporary profiles cannot exceed 12 months.
If a profile is needed the Soldier must complete a profile request packet and send packet to usarmy.usarc.usarc-hq.mbx.armmc@mail.mil (AR-MMC mailbox). It is also the Soldiers responsibility to report medical conditions that effect medical readiness to their commander.
