Purdue University Student Health Service. The flu vaccine is a requirement for all Purdue students. To be compliant with this requirement, you must upload proof of your flu vaccine to the link located on the homepage of your Patient Portal. Proof of the flu vaccine or declination is due by October 31st.
Students over 18 will submit their dates online in the patient portal. Students under 18 will provide PUSH with either a record of their vaccines or completed immunization history form. After submitting immunization dates, schedule a PMIR visit, which is a Pre-Matriculation Immunization Review.

All registered West Lafayette campus domestic students taking six or more credit hours and degree seeking are eligible to enroll in the University Injury and Sickness Insurance Plan. All qualified Fellows on the West Lafayette campus are eligible to enroll in this insurance plan.
