Great tool.
Great tool. Only suggestion is being able to use keyboard arrows to align text. I have shakey hands and had difficulty aligning text.

You can send us a letter or use our standard form, Claimant's Appointment of a Representative (SSA-1696). The form is available from our website at www.ssa.gov/forms/ssa-1696.pdf or at any local Social Security office. You can submit the form online or by mail.
This service allows you and the individual you agree to represent (i.e., the claimant) to complete your respective sections of the notice of appointment (Form SSA-1696) online, sign the form electronically, and submit it to us electronically.
Usually a family member will be the person to apply to be your representative payee. At the time when someone other than you needs to communicate with Social Security on your behalf, there is a process for that individual to be appointed as representative payee.
The representative may petition for a fee. A named third party will pay the representative a fee equal to the lesser of 25 percent of the past-due benefits or the specified dollar amount (e.g., $6000), and the claimant will have no financial liability for paying the authorized fee.
The SSA-1696 is a document that allows a claimant (someone who wants to apply for benefits) to appoint a representative to assist them with their application process.
