How should I print the 1500 Form for insurance so that it is properly aligned?

This is the first time I have used PDFfiller. I was using the 1500 Form for Insurance. When I tried to print onto the form it printed it was not in alignment. How do I get it to it to line up printing correctly?

Dear User, Thank you for contacting PDFfiller support. Please fill the form that is available from this link: www.pdffiller.com/100390300-1500_CMS_02-12pdf-- Then click "Done" button and choose the option "Save as PDF" to download the filled form to your computer. Select the required pages and make sure to click the option "Print content only" before you save it to your computer. Now open the form that was saved on your computer using Adobe Reader software and print it by selecting the "Actual Size" option in "Page Sizing & Handling" section of the Print dialog box.
