I can't print my CMS 1500 form. What should I do?

Hello, I have tried printing my CMS 1500 medical claim form several different ways on the official red paper and the alignment never ends up correct. Are there any pointers that you have as to how to remedy this?

Dear User, Thank you for using PDFfiller.com. Printing on a pre-printed form may cause alignment issues because the online version of the form may be different than the one you have. We suggest printing the entire form instead of only the content that you filled. That way you won't find any alignment issues. You can also scan your form in exact size as PDF file then upload to PDFfiller, fill and print on top of your original form.
