Click on the 'Adapt Checkbox Attestation' button in the toolbar.

A pop-up window will appear with options for customizing the attestation checkbox. You can choose the label text, font size, font color, and checkbox size.

Once you have customized the attestation checkbox, click on the 'Insert' button to add it to your document.

The attestation checkbox will be added to your document. You can move and resize it as needed.

To edit the attestation checkbox, simply double-click on it. The pop-up window will appear again, allowing you to make changes to the label text, font size, font color, and checkbox size.

To remove the attestation checkbox, right-click on it and select 'Delete'.

This takes the process ahead of just creating a Microsoft Office form, but allows you to create sophisticated forms that can be filled and even electronically delivered. Often the layout gives me trouble or won't let me create form boxes in the appropriate area because it keeps snapping them to the grid, which might not automatically line up to my form fields as well.
