I would prefer that the fill in templates be free of charge. The one used is great and I would hope to find another one as such. Easy to use formatting and site.

First, open the PDF document and provide the password it requires. Click the Print button on the PDF toolbar while viewing the password-protected document. Click the Change button under Destination and select Save as PDF.
