They don't take away your authority to make your own care and treatment decisions. You retain the right to override the decisions or your representative, change the terms of your living will or POA, or completely revoke an advance directive.
Naming a health care agent proxy does not take away any of your authority. You always have the right, while you are still competent, to override the decision of your proxy or revoke the directive. Choosing a health care proxy is the most important decision you will make.
It's absolutely essential for anyone who is 18 years old or older. Some (but not all) states have laws to cover a patient who hasn't designated someone to make health care decisions. Such laws contain a priority listing of those who can make decisions for an incapacitated patient.
Federal law does not require individuals to complete any form of advance directive (and nor do state laws), and it expressly forbids requiring an advance directive as a requisite for treatment.
What Happens If I Don't Make an Advance Directive? You will receive medical care regardless of whether you have an advance directive. If you cannot speak for yourself and do not have an advance directive, a physician will generally look to your family, friends, or clergy for decisions about your care.
If you don't have a medical power of attorney, then someone who you would not have chosen might end up making medical decisions for you. The only situation where someone else could make medical decisions for you is when you are incapacitated.
When You are Unable to Make the Decision Due to illness or injury, you may not be able to state your wishes about CPR. In this case: If your doctor has already written a DNR order at your request, your family may not override it. If so, this person or a legal guardian can agree to a DNR order for you.
If you attempt resuscitation on a person who has a valid DNR order, you open yourself up to lawsuit by the estate (read: family) of the person with the DNR order, especially if the attempt is successful, and they are indeed brought back to life.
A prehospital DNR order is generally a simple, one-page document; you don't need a lawyer to prepare it. You do, however, need to talk to your physician, who will sign your DNR. In some states, adult witnesses or a notary public must also watch you sign the order.

To be valid, an advance directive must be signed by you in the presence of two witnesses, who will also sign. The person you name as a health care agent may not also be a witness. Some states also require the advance directive to be notarized.
