Go to Insert > Form Items > Radio Button Group. This opens the Insert Radio Button Group window. Enter a name for the group. This will appear in your form results. Enter your radio button items. Click Add to add more item-value pairs to your list. Click OK.
Select the grouped object and press Ctrl+Shift+G. Right-click the grouped object, scroll to Group, and select Ungroup. Select the grouped object. Go to the Home tab, click Arrange, and select Ungroup.
Begin by creating an input element to serve as the basic foundation. Set the type to radio. Give each radio button a unique id. Use the name attribute to identify all the buttons in a group. Consider visual grouping as well.
The best way to align radio buttons is using the vertical align middle CSS with margins adjusted on the input elements. I like putting the inputs inside the labels (added bonus: now you don't need the for attribute on the label), and put vertical-align: middle on the input. (The -2px margin-top is a matter of taste.)
