With the table that contains your attachment field open in Data sheet view, double-click the attachment field. Click Add. Use the Look in list to navigate to the file or files that you want to attach to the record, select the file or files, and then click Open.

In Microsoft Forms, open the form you want to add a picture and then select the form header section. Click Insert Image . You can choose to search for pictures and images from Bing, your OneDrive folder, or upload a picture from your computer or device.
In the Navigation pane, select the table you want to use to create a form. Select the Create tab, locate the Forms group, and click the Form command. Your form will be created and opened in Layout view. To save the form, click the Save command on the Quick Access toolbar.
