Does what I need, the only thing is, it can be confusing when you try to log in, it takes you to another page instead of your actual account page so I have to search for something else to get to it, or I may just haven't located how yet, still good for what I need done.

Pretty easy to use! Pretty easy to use!Update: Been using this a while now, only down side is that when you blank off certain information, save the file and open it to check, for a very brief second all the old information flashes up.

You do, however, need to talk to your physician, who will sign your DNR. In some states, adult witnesses or a notary public must also watch you sign the order. A prehospital DNR order won't have any effect unless emergency medical personnel see it before they begin CPR.
You should be able to easily reverse a DNR regardless of your location. Simply tell a doctor or nurse that you would like to revoke your DNR order and make a different plan for emergency services. Your physician will document your wish to be resuscitated in your medical records. You may have to sign paperwork as well.
The doctor writes the DNR order on your medical chart if you are in the hospital. Your doctor can tell you how to get a wallet card, bracelet, or other DNR documents to have at home or in non-hospital settings. Standard forms may be available from your state's Department of Health.
Connecticut's DNR Program CCP has run the state's Do Not Resuscitate Program since 1991. Bracelets are available through the organization and must be ordered by a licensed physician or institution using the order form in the drop-down menu on this website.
Out of hospital DNS do not expire. The Out of Hospital DNR form and/or bracelet should be taken with the patient.
A DNR can also be valid if: (1) it is not contrary to the directions of a patient who was competent at the time the patient conveyed the directions; (2) in the reasonable medical judgment of the patient's attending physician, the patient's death is imminent, regardless of intervention of CPR; and (3) the DNR order is
They can be included as part of advance directives or advanced healthcare directives. A DNR can also be included in a living will. You can create a DNR online with a template or seek legal counsel to draft one for you.
A prehospital DNR order is generally a simple, one-page document; you don't need a lawyer to prepare it. You do, however, need to talk to your physician, who will sign your DNR. In some states, adult witnesses or a notary public must also watch you sign the order.
A DNR or Do Not Resuscitate request is usually made by the patient or health care power of attorney and allows the medical teams taking care of them to respect the patient's wishes. In the U.S., CPR and advanced cardiac life support (ACLs) will not be performed if a valid written “DNR" order is present.
Medical professionals who give CPR to people with a DNR order can potentially be in trouble if they know about the DNR. This means that an emergency response team can legally give someone CPR even if they have a DNR order.
Is a DNR a Good Idea for You or a Loved One? One might think that signing a Do Not Resuscitate (DNR) order will prevent this from occurring.
