PDFfiller Review Great and easy way to make sure your PDFs are signed, sealed, and delivered as quick as possible. As a contract administrator for my company, this makes signing PDFs easy, and allows it to be done in a timely manner. Easy to navigate, allows you to download your finished, signed PDF for storage. Takes a little bit of time to learn the advanced functions, and a variety of color coding features who make highlighting PDFs more efficient when needed/if multiple people are adding notes.

Below is a list of the most common customer questions. If you can’t find an answer to your question, please don’t hesitate to reach out to us.
Events leading up to the incident. Events involved in the incident. Events immediately following the incident.
Accurate. All data must be clear and specific. Factual. An incident report should be objective and supported by facts. Complete. Ensure that all essential questions (what, where, when, why and how) are covered in the incident report. Graphic. Valid.
Ibuprofen Dosage Take ibuprofen with food or after meals if it seems to bother your stomach. Prescription doses of ibuprofen normally range between 400 mg to 800 mg, up every six to eight hours. In those cases, you should not take more than 3.2 g or 3,200 mg daily.
OSHA recordable is a term for injuries and illnesses that must be reported to the Occupational Safety and Health Administration (OSHA) on a Form 300 (Log of Work-related Injuries and Illnesses). It includes a work-related injury or illness that results in any of the following: Medical treatment beyond first aid.
Recordable vs Reportable Events. OHSA reportable events cover fatal or extremely serious injuries or illnesses. In addition to being recordable events, the following must be reported to OSHA within specific time limits. Any work-related fatality must be reported within 8 hours.
You recorded the case based on the days away from work, and ask whether the entry may now be lined out because the MRI showed that no OSHA recordable injury occurred. Accordingly, the MRI results are not a basis to line out the entry. Thank you for your interest in occupational safety and health.
Whether a foreign body is found, a medical provider may elect to instill one dose of antibiotic eye drops. The treatment of this case clearly involved the use of two doses of prescription medication and is therefore considered medical treatment for OSHA record keeping purposes (page 43, Bluebook).
Although OSHA does not specifically mention pink eye in any of its recordkeeping-related materials, you may find the following example given in Detailed Guidance for OSHA's Injury and Illness Record keeping Rule helpful in making the determination whether the infection is work-related.
See, 66 Federal Register 5984. We note that Benadryl and Analog injections are not included on the list of first aid treatments set forth at 1904.7(b)(5)(ii). However, once medical treatment is provided for a work-related injury or illness, the case is recordable.
Basic requirement. You must consider an injury or illness to meet the general recording criteria, and therefore to be recordable, if it results in any of the following: death, days away from work, restricted work or transfer to another job, medical treatment beyond first aid, or loss of consciousness.
Response: The administration of oxygen is considered medical treatment for OSHA injury and illness record keeping purposes. If an employee is symptomatic of a work-related injury or illness (e.g., respiratory distress from smoke inhalation) and is administered oxygen, the case is recordable.
You must consider an injury or illness to meet the general recording criteria, and therefore to be recordable, if it results in any of the following: death, days away from work, restricted work or transfer to another job, medical treatment beyond first aid, or loss of consciousness.
Is this recordable?” Answer: Yes. Chipped or broken teeth are always recordable. OSHA considers these cases to be significant injuries, even if they don't meet any of the other recordable criteria such as days away from work or medical treatment beyond first aid.
Answer: Yes, this case would need to be recorded on the OSHA 300 log as a lost-time incident. A case in which a physician or other licensed health care professional recommends that an employee takes one or more days away from work must always be recorded, regardless of the ultimate severity of the injury or illness.
