Choose a document in your pdfFiller account and click signNow.

Add as many signers as you need and enter their email addresses. Move the toggle Set a signing order to enable or disable sending your document in a specific order.
Note: you can change the default signer name (e.g. Signer 1) by clicking on it.

Click Assign fields to open your document in the pdfFiller editor, add fillable fields, and assign them to each signer.

Easy and economical I have used it to create and edit pdf documents for over 10 years now. Its ease of use. I tried it after a web search when I needed to do some general edits and create some pdf files and I didn't want to spend a ton of money for Adobe. This program was just the ticket. A multi-part CCF form (AADT's Mentor CCF is 5-parts) and other supplies are used to complete the CCF process. Typically, for DOT testing there are 5-parts or copies to a federal CCF.
Chain of custody (COC), in legal contexts, is the chronological documentation or paper trail that records the sequence of custody, control, transfer, analysis, and disposition of physical or electronic evidence. It is often a tedious process that has been required for evidence to be shown legally in court.
The Chain of Custody Form (CCF or COC) is used to record all changes in the seizure, custody, control, transfer, analysis, and disposition of physical and electronic evidence. A typical Chain of Custody Form will describe the evidence and detail the location and conditions under which the evidence was collected.
For DOT tests, our MRO tries to reach the donor 3 times within 24 hours. If no contact, they let us know, and we'll ask you to have him/her contact the MRO. At that point, if they don't hear from him, they release results in 72 hours.
(a) When, as the MRO, you receive a confirmed positive, adulterated, substituted, or invalid test result from the laboratory, you must contact the employee directly (i.e., actually talk to the employee), on a confidential basis, to determine whether the employee wants to discuss the test result.
