If you do not hold a federal medical card or are not grandfathered, you will be issued a restricted commercial driver license.
You do not have to keep your medical certificate on file with WA SDL but you must carry it with you when operating a CMV. Per RCW 46.25. 055 CDL drivers must carry Medical Examiners Card in Washington.

The CDL holder will then have 45 days from the mail date of the notice to provide a new Medical Examiner's Certificate (DOT Physical Card) to avoid being downgraded and having the commercial designation removed from their driver's license.
The cards are valid for a maximum of two years. This applies even if the driver never leaves the state. An exception applies to grandfathered drivers – those who had a Wisconsin CDS prior to July 29, 1996.
