What is a DS-11 form?

Every US citizen who intends to go outside of the country needs to have a US passport. To issue the document, a US citizen fills out a DS-11 form providing all the required information. Once the submission has been processed and approved, the applicant becomes the proud owner of a 28-page passport book, 52-page passport book, or a passport card — they can choose which type they prefer at the very beginning of the DS-11 form.

Keep in mind that passport cards can only be used for traveling to Mexico, Canada, the Caribbean, and Bermuda. Passport books are valid for travel to all countries.

Other important information:

For applicants sixteen years of age or older, an issued passport is valid for ten years.

For applicants under sixteen years, an issued passport is valid for five years.

If you need your passport urgently, contact the nearest passport agency.

Dates: There is no official date for submitting an application. Just keep in mind that it may take up to ten weeks to process your DS-11 form.

Eligibility requirements: You can fill out the application personally if you are at least sixteen years old and can prove that you are a US citizen or non-citizen national.

How to fill out your DS-11 form

Filling out the DS-11 form is straightforward and easy. The application has two pages for completion. You have to provide your personal information, add your color photo, and sign the form.

Fill out your name, date of birth, social security number, mailing address, marital status, information about your most recent US passport, etc.

Add your color photo to the form. Follow all the requirements listed.

Fill out parental information (if applicable).

Add emergency contact information.

Sign the form in front of an authorized acceptance agent in your local passport office, passport acceptance facility, or USPS office.

Pay application and processing fees.

To prove your citizenship, you need to provide the following:

A certified birth certificate (for applicants born in the US or US nationals).

A consular report of birth abroad, aka FS-240 form (for applicants born outside of the US).

A certificate of naturalization or certificate of parent’s naturalization (for applicants born outside the US who are naturalized).
