If you've already changed your name, you'll need proof of such to update documents and identification like your driver's license, passport, and social security card. You'll need to update all identification documents to ensure you have the same name on each document.
Certificate copies certifying a legal name change are available through your county vital records or public health office. Once your name is legally changed, you need certificate copies to update your name with schools, banks, employers and public agencies.
You can get a certified copy of your order by visiting the Clerk of Court's Records Department. You can request a new birth certificate to be issued with the new name as part of your final name change order. If so, you will need to contact the vital records department where you were born to find out their requirements.
How many should I order? We recommend that you order 10 additional certified copies of your deed poll as there are many business and companies that you need to inform. As there are many people you need to notify of your name change, you don't want to send your original Deed Poll in the post in case it gets lost.
All name change applications can be viewed by anyone because they are public records. Also, the Court will require notice of the name change to be printed in a newspaper. A sealed name change request cannot be found in the public records. The Court file will stay sealed while the Court considers the request.
Fill out your court forms. Have your forms reviewed. Make at least 2 copies of all your forms. File your forms with the court clerk. Publish the Order to Show Cause for Change of Name (If Required) Is incarcerated or on parole, serve the documents. Go to your court hearing, if necessary.

All applicants for a driver license and/or identification (DL/ID) card must submit his/her social security number to DMV. You will receive a Request for Verification of Information letter from DMV informing you that the information you provided (name, birthdate, and/or SSN) does not match SSA's records.
