Worth the money! The software is very easy to use from a smartphone or computer. Most people don't own a fax machine, so this feature is very handy! The subscription is necessary, but now with many places of business using editable PDFs, it's not needed as often as I'd like.

Calories's Form Tracking feature allows you to easily stay organized and on top of every form submitted to your website. Each time a customer fills out a form, we'll capture the contents of that form and instantly alert you by phone call, text message, or email. This allows you to increase your lead conversion rate.
Once you've enabled form tracking, you can review form activity and data in your Google Analytics account. Google Analytics can record anything from page views to button clicks. To find event tracking in your Google Analytics account, look at the left sidebar. Go to Behavior > Events > Overview.
You can create both anonymous and non-anonymous (identified) surveys on Google Forms based on your requirements. If you want to create an anonymous survey, you can leave the option Collect email addresses unchecked. And the end-user can take the survey without giving his/her email address.
They are submitting multiple entries, and because Google Forms will not record the I.P. If you are a Google Apps user, you can always restrict the Google Form to accept entries only from users who are part of your domain and the response spreadsheet will then record the username of the form submitter.
