I looked through all of the court libraries, many online results, etc to find what I needed to represent myself in court against my ex to defend my parenting rights, and this was the only place a found the form I needed, that was writable and downloadable. It took a like while for me to figure out how to do what I wanted to do. But once I did, I was able to create the doc I needed. The part I liked best was the guidelines to put the fillable boxes in line with the other boxes in the document.
