Had many of the forms I needed, however, surprised with researching NC business forms I found a 1997 form but not a 2000 form. Fortunately I can use a 2001 form for 2000 year.

How hard is it to get into OT school?
It IS hard to get into OT programs unless you are able to stand out in some way above other applicants. I say this because in the school that I was accepted into there were over 300 applicants for 30 positions. And this is supposedly not that unusual. All the applicants met minimum GPA and other requirements.
How do I succeed in OT school?
Give yourself a pat on the back and enjoy whatever time off you have before embarking on the next journey. Mentally prepare. Find your study buddies. Be as well-rounded as possible. Learn to manage your finances. Find a hobby.
Is OT school easy?
These courses are almost always in the beginning, which helps ensure that admitted students can handle the rigors of graduate school. While it's not easy, the OT school curriculums do a good job of providing you a solid foundation as you begin your occupational therapy career.
What is the average GPA to get into occupational therapy school?
For example, the average GPA of students admitted to INPUT's Department of Occupational Therapy is 3.7/4.0 and the minimum required to apply is 3.0/4.0. Volunteer and Clinical Experience — Most schools will require a certain number of hours volunteering or working under a licensed OT.
Is becoming an OT worth it?
Is becoming an occupational therapist worth the cost? The financial answer is yes, but there is a significant price to pay. For most occupational therapists, they will have to deal with student loans for 10 to 20 years won't necessarily be able to celebrate and enjoy that higher income until their loans are dealt with.

Yes, occupational therapy is one of the fastest-growing careers, you get to make a difference in other peoples' lives, and the pay is good. Also, OT's can work in a wide variety of settings, meaning there are always work opportunities.
Is occupational therapy a stressful job?
The short answer is no, for a variety of reasons that I'll get into. However, I also don't think that OT is special because of it. If you've never had a real job before, it's important to know that most jobs, going to school, and parenting are all pretty stressful. Life is stressful!
