It's an awesome program. The only problem is that this form is used with all insurance companies and the top should be blank for that particular company to fill in their name.

I have to tell my dr. to use this. Tired of his hand-printed & copied over & over stuff & can fax it immediately since he snail mails everything. Highly recommend to anyone who can use all the features. Added plus is that it's so easy to use. It eliminates paper, toner and postage and well worth the price for anyone who needs all the bells & whistles (which he does!!). Thank you, PDFfiller !!

How do you send a PDF with Sticky Notes?
Create Sticky Notes for your document. Open your document in Acrobat's online PDF editor. Select the text where you'd like to add your note or comment. Click Add Sticky Note (Ctrl+6). Make your comment and repeat throughout the document. Save your edited document.
Is it possible to print PDF with comments?
Acrobat X, Reader Choose File > Print. In the Print dialog box, click the Summarize Comments button. Acrobat opens a separate PDF of the pages and summarized comments. You can then print this PDF.
How do I share a PDF with Sticky Notes?
Open the PDF with Adobe Reader, click the Add Sticky Note button on the toolbar and click in the PDF where you wish to insert this icon. You can also click the Comments link in the right corner to open the Annotations pane and then click Sticky Note. A dialog box with your name, date and time will display.
