It´s definately a good pdf writing and editing service, but they do charge quite a lot for people who only use it casually, like me, maybe they should have to more payed plan options...

What is a wage affidavit?
The Employer Wage Affidavit and Information Sheet is for the purpose of providing the court with information and records concerning a party's income and benefits to assist the court in making decisions regarding that party's pending case.
Who is required to submit certified payroll?
What is certified payroll reporting? Davis Bacon requires contractors who win a bid on a federally-funded construction contract over $2,000 to pay their laborers the local prevailing wage and fringe benefits for their work, and to complete a certified payroll report for those employees.
Is certified payroll the same as prevailing wage?
Projects that are publicly funded typically require the contractor to pay a prevailing wage (Davis-Bacon wage) rate for workers on the job. Then, for each payroll period, the contractor must create a certified payroll report that shows the wages paid to each worker.
What does certified payroll look like?
Certified payroll is a federal form WH-347, submitted weekly to the agency overseeing a government contract. The form lists every employee, their wages, the benefits they're entitled to, the type of work they did, and the hours they worked. It shows withholding and gross wages and includes a statement of compliance.
Can QuickBooks do certified payroll?
For now, the Certified Payroll feature is only available in QuickBooks Desktop. You might want to run the different payroll reports (Payroll Details, Payroll Summary by Employee, Payroll Tax and Wage Summary, and Time Activities by Employee Detail) in QuickBooks Online to get amounts of wages paid to your employees.

How do I do certified payroll in Quickbooks?
Go to the Reports menu. Choose Employees & Payroll. Go to More Payroll Reports in Excel, then choose New! Certified Payroll Report. Follow the on-screen instructions to create the report.
What is a statement of compliance for certified payroll?
The signed statement of compliance asserts that the attached payroll report is complete and correct. The statement confirms that all covered workers have been paid as required by the Davis-Bacon Act. This is always due no more than seven days after the regular pay date for the pay period that the report covers.
