I have just been informed by my college that I can't use this format. After paying for it and working with it for months, they prefer I use a format with expandable boxes for each indicator. This is a user friendly method but my only difficulty as been that the only information I can add to the PDF is what fits in the fixed boxes. This would be a suggestion in the formatting of this PDF.

Quick to respond to email.Most of the time when you send an email, it goes out and either no response or very delayed response. NOT so with PDFiller, they responded to my question quickly and took care of the issue. I am very happy with the response as well as how efficient they were. This is a great program if you need this service. We needed it at one time, but did not realize we could do this on line. I would recommend it to anyone who is in the business of needing documents filled out, signed and delivered.

At first it seemed hard, trying to figure out how to save the completed document, and to get to the next step of saving the document and sending it by email. But I got it now. Thanks
It was a little challenging at first and the online help was a little overbearing to start. You should always assume that people know how to operate every aspect of there computer. I had a glitch and he was a little belittling to me which being 50 and having diabetes its hard for me to see sometimes and once i stated that his demeanor changed and he helped me finally.
