It's an awesome program. The only problem is that this form is used with all insurance companies and the top should be blank for that particular company to fill in their name.

I had issues with the account and she was eager to fix it and then I had issues with the form and the same excellent response. She was not only knowledgeable, but fast and very, very helpful. much better than trying to fill out…
much better than trying to fill out some of these forms by hand or create a form from image using something like Adobe acrobat. Only problem was you should probaby do better quality control on the forms provided. I found your Chapter 7 petition package to be unusable (all or most of the checkboxes use the same variable i.e., check one and you check them all.

I love this program
I love this program! I am able to easily create PDFs and sign them with a verified signature. It just showed me how to merge two documents into one. Great program!
