What do you like best?
We like PDFfiller because our patients don't want to have to waste precious minutes of their appointment time completing a health history update in the office. It's much simpler, not to mention faster, to have them do it at home and submit it when they're done!
What do you dislike?
The log-in process for the patient who has never used this service. We have a few patients who value their privacy and would rather not have to create an account in order to complete a form.

Owners must submit the previous title or Manufacturer's Certificate of Origin, a $15.00 fee, and a bill of sale if the title does not have the Bill of Sale on the back. No sales tax is required on mobile homes if it was purchased between individuals. The title fee is $15.00.
The owner or seller will contact the DMV or state agency in charge of mobile home titles and file for a lost or duplicate title request. In order to get a replacement title, the owner will need to prove ownership and that all taxes are paid.
A $95 fee will be required when transferring title. Buyers must show proof of Illinois residency. If for any reason a lien needs to be placed on the title, this will be done at the DMV while talking with the clerk that is helping you.
A title is not required; however, if a homeowner is converting their home to real property. You will need a certificate of title in order to finance the purchase of a manufactured home and to have proof of your ownership of the home.
An applicant for a duplicate certificate of title for a mobile home must file an affidavit describing the loss along with the application. A duplicate certificate will not be issued until 11 days after the application is filed.
